Obtaining and Maintaining DBID Credentials

Obtaining and Maintaining DBID Credentials for JAX Military Base access.

  1. Email the Administrative Assistant for the Navy Exchange NEX Jacksonville, Evelyn Campbell, at evelyn.campbell@nexweb.org for an updated 5512 Form (Department of the Navy Local Population ID Card/Base Access Pass Registration).
  2. Review DBIDS Registration PowerPoint
  3. Complete or update the NAS JAX DBIDS Company Enrollment spreadsheet.
  4. Complete tabs:
  • Enrollment form –
    • Company information – ADV contact information.
    • Contract information – “Annual Contract”, begin date 01-01-202X, and end date 12-31-202X.
    • Days and times of access – select Mon-Sun 05:00 – 18:00
    • Sponsor information – Evelyn Campbell, Navy Exchange NAS Jacksonville, 904-777-7200, evelyn.campbell@nexweb.org.

  • Employee – list new and current employees
  • Add New Employees – list any new employees to have access to the base
  • Terminated Employees – any employees that need access to terminated
  • Lost or Stolen IDS Card– any employee(s) who need new credentials due to a loss of their access card

6. Email Form 5512 to each employee listed on the spreadsheet, instructing them to complete all highlighted areas, sign, date, and provide a copy of their SS card and State Drivers License.

7.  Email Evelyn Campbell, the Company Enrollment spreadsheet, the 5512 form for each employee, and their supporting documentation.

8.  Once approved, Evelyn will email instructions on how the employee needs to proceed to the base to obtain the ID card.

Once DBIDS Credentials have been established, the credentials will need to be renewed each December regardless of the issued date. Follow the steps above.
